Refer Yourself to Our Practice
There is no need for you to be referred by your own general dentist. We will accept a ‘self referral’. However, we will require certain personal details from you in order for an appointment to be made – your name, date of birth, address, telephone numbers (landline and mobile), e-mail address, and how you heard about us.A self referral can be made either by telephoning the practice or sending the online Self Referral Form to use via the internet.
